The Office of Federal Contract Compliance Programs (OFCCP) announced that contractors must begin to use the revised Voluntary Self-Identification of Disability Form starting August 4, 2020.
What Has Changed?
- The revised form has been consolidated from two pages to one and is currently available in English. The OFCCP has indicated that the form will be available in additional languages.
- The form now includes references to the annual disability 7 percent utilization goal, Section 503 of the Rehabilitation Act, and includes a link to the OFCCP website.
- The “Reasonable Accommodation Notice” section has been removed.
- The list of sample disabilities has been expanded to include, autoimmune disorders, low vision, celiac disease, psychiatric conditions, and anxiety, among others.

The revised form (CC-305) must be used without any alterations to the content and expires on 5/31/2023. Contractors must invite applicants and new hires to self-identify as an individual with a disability using this form.
At a minimum, contractors must invite their employees to self-identify every five years, beginning the first year that they become subject to the Section 503 voluntary self-identification requirements. In addition, at least once during the years between these invitations, contractors must remind their employees that they may voluntarily update their disability status at any time.
Through the invitation and reminder to employees to self-identify, contractors can capture data on employees who become disabled while employed, as well as those with existing disabilities who may feel more comfortable self-identifying once they have been employed for some time. It also allows contractors to monitor and improve their practices regarding placement, retention, and promotion.
Contractors may not compel or coerce individuals to self-identify and must keep all self-identification information confidential.
